Why does an upper molar toothache sometimes feel like sinus pressure? The roots of the upper molars (especially first molars) sit just millimetres below the floor of the maxillary sinus. Inflammation around the tooth root can irritate the nearby sinus lining, and conversely sinus congestion can make upper teeth ache. This shared neighbourhood is why dentists check both tooth and sinus when upper back teeth hurt.
