A teenager with fever, neck stiffness, and a non-blanching rash gets ceftriaxone immediately. Meningococcal meningitis (Neisseria meningitidis — kidney-shaped diplococci) kills in hours — petechial/purpuric rash (doesn't fade on pressure — glass test) + meningism means treat before lumbar puncture: IV ceftriaxone + chemoprophylaxis for contacts (rifampicin/ciprofloxacin). Vaccine (quadrivalent) prevents. The rash that doesn't blanch is the emergency's signature — minutes matter.
