A marathon runner collapses at 38 km on a hot humid day, confused with hot dry skin. This is heat stroke — core temperature above 40°C with brain dysfunction; sweating may have failed (hot dry skin). Muscles generate enormous heat; humidity blocks evaporation, and dehydration cripples sweating. It kills by multi-organ failure — immediate aggressive cooling (ice water immersion) saves lives, faster than any drug. Prevention is hydration, acclimatization, and pacing — physiology applied.
