During pneumonia, the alveoli fill with fluid instead of air — oxygen cannot diffuse into blood, and patients gasp despite breathing hard. Smokers' tar paralyzes the airway cilia, so mucus and dust accumulate, causing the chronic 'smoker's cough' as the lungs struggle to clean themselves.
