A wound is sutured under tension and breaks down. What principle was violated? Tension-free closure - wound edges approximated without pulling; tension strangulates blood supply and the wound dehisces. Students learn suturing's gentleness rule: approximate, don't strangulate - the suture holds edges together while biology heals; tight sutures fight the healing they were meant to help.
