A midline diastema has a thick frenum inserting at the papilla. Why address the frenum first? The frenum's fibres maintain the gap - closing it orthodontically without frenectomy invites relapse; blanch test confirms. Students learn cause-before-cosmetics: diagnose why the gap exists (frenum, habit, Bolton discrepancy, missing lateral), treat the cause, then close - because the closed gap that relapses was never really treated.
