Study notes
Patient: 32-year-old male. Presentation: Reports painful swallowing and white patches on tongue. Examination: Intraoral exam shows creamy white plaques on buccal mucosa that rub off with 2x2 gauze, leaving red, raw spots (Pseudomembranous Candidiasis). Step 1: Perform a brush biopsy to confirm fungus. Step 2: Prescribe Clotrimazole 10mg troches (lozenges) to be dissolved in mouth 5 times daily for 14 days. Step 3: Refer to physician for CD4 count check. Follow-up: Patient returns after 14 days; plaques are gone, but patient is referred to a specialist for long-term ART management.
