A ventilated patient's fever spikes on day 5 — VAP suspected; cultures drawn. Ventilator-associated pneumonia (after 48h intubated) is prevented by the bundle — head-end 30–45°, oral chlorhexidine, daily sedation breaks + weaning assessment, DVT/ulcer prophylaxis. Diagnosis — new infiltrate + fever + purulent secretions; cultures before antibiotics. De-escalate by culture. The ventilator saves breath but invites bacteria — the bundle is the chaperone.
