A wound is on the front of the forearm, 5 cm above the wrist, on the thumb side. Anatomically: '5 cm proximal to the wrist, on the anterior aspect of the forearm, radial (lateral) side.' Using standard terms, any doctor anywhere understands exactly where — no confusion, no pointing needed.
