Calf pain and swelling after a long flight — DVT on Doppler; heparin started. DVT (Virchow's triad — stasis, hypercoagulability, endothelial injury: calf pain, swelling, Homan's: Doppler — non-compressible: heparin/LMWH + warfarin/DOAC 3–6 months; stockings; IVC filter if anticoagulation contraindicated; PE — chest pain, dyspnea — CT pulmonary angiography — emergency). Varicose veins (valvular incompetence — dilated tortuous — standing: stockings, sclerotherapy, laser ablation/stripping). PAD (atherosclerosis — claudication — ABI <0.9: cilostazol, exercise, statins; angioplasty/stent/bypass; critical — rest pain, ulcers, gangrene — revascularize). AAA (>3cm — >5.5cm repair — open/EVAR; ruptured — emergency). The swollen calf after travel is DVT — Doppler confirms.
