After thyroid surgery, a patient speaks in a hoarse whisper. The recurrent laryngeal nerve runs in the groove between trachea and esophagus, looping under the aorta on the left, and supplies the vocal cord muscles. Thyroid surgery happens right next to it — if bruised or cut, the vocal cord on that side can't move, and the voice turns hoarse. Surgeons identify and protect this nerve in every thyroid operation.
