Nocturnal itching, burrows on wrists — scabies; permethrin; family treated. Scabies (Sarcoptes — nocturnal itch — burrows — wrists, genitals: 5% permethrin — neck-down — overnight; repeat 1 week; treat contacts — ivermectin — crusted): pediculosis (lice — head — nits: permethrin 1% — comb; body — hygiene; pubic — shave + permethrin). HIV dermatology (seborrheic dermatitis, recurrent herpes/zoster, Kaposi — HHV-8 — violaceous — ART + chemo; molluscum — giant). The nocturnal itch is scabies — treat the household.
