A woman — low mood, no interest, 3 weeks; depression; SSRI + counseling. Depression (low mood + anhedonia — 2 weeks + 5 symptoms — sleep, appetite, guilt, concentration, suicide: treat — SSRIs — fluoxetine/sertraline + CBT; severe/psychotic — ECT; suicide risk — ask directly — hopelessness, plan, means — admit): bipolar (mania — elevated, grandiose, decreased sleep, pressured — 1 week; hypomania — 4 days; depression: treat — mood stabilizers — lithium — monitor levels/thyroid/kidney; valproate; mania — antipsychotics + benzodiazepines). The 2-week anhedonia is depression — ask about suicide directly.
