A young man — hears voices, believes persecution; 8 months; schizophrenia; risperidone. Schizophrenia (delusions — persecutory; hallucinations — auditory; disorganized — 6 months + dysfunction: positive — delusions/hallucinations; negative — flat affect, apathy; cognitive: treat — antipsychotics — risperidone/olanzapine — first-line; clozapine — resistant — agranulocytosis — monitor; side — EPS — parkinsonism, metabolic — weight/diabetes): delusional disorder (single non-bizarre delusion — functioning preserved — antipsychotic low dose), brief psychotic (<1 month), schizoaffective (mood + psychosis). The 6-month dysfunction is schizophrenia — antipsychotics are the mainstay.
