Neonatology: neonatal sepsis, respiratory distress, birth asphyxia
Pediatrics · MBBS
Study notes
A 3-day-old — lethargic, not feeding, hypothermic; sepsis; blood culture, ampicillin + gentamicin. Neonatal sepsis (early <72h — maternal; late — nosocomial: lethargy, poor feeding, temperature instability, apnea, jaundice: blood culture + CRP; empiric — ampicillin + gentamicin; meningitis — bulging fontanelle — LP — cefotaxime): treat fast — deterioration is rapid. RDS (preterm — surfactant deficiency — grunting, retractions, tachypnea — X-ray — ground glass: CPAP + surfactant). Birth asphyxia (failed to cry — APGAR low: resuscitate; HIE — therapeutic hypothermia — cooling — neuroprotection). The lethargic unfed newborn is septic — culture and treat immediately.
