An elderly man — acute confusion, fluctuating; UTI found; delirium; antibiotics, confusion clears. Delirium (acute — fluctuating attention — CAM: cause — infection, drugs, metabolic — treat cause; manage — reorient, haloperidol low): dementia (chronic progressive — Alzheimer's — memory — donepezil; vascular — stepwise — control HTN; Lewy — visual hallucinations — avoid antipsychotics; frontotemporal — behavior: MMSE/MoCA; BPSD — agitation — non-pharmacological first). Depression — pseudodementia — reversible — screen. The acute fluctuation is delirium — find the cause.
